Providing individual care and support services, built with the recipient to enable them to flourish and integrate into the wider community.
We will provide an in depth needs assessment and a bespoke package of care for every individual using our service.
By assessing the appropriate risks and having access to enhanced support, we are able to provide a very vulnerable group of people with the opportunity to live in their own home rather than living in a residential care home.
Designing a care and support package which will enable our service users to lead a fulfilling and enjoyable life.
At Clear Pathway Care we specialise in supporting individuals with severe learning disabilities, autism and complex behaviours that may challenge others.
Highly trained staff are provided with ongoing support and training to provide an excellent level of care.

Hi I’m Jared, I founded Clear Pathway Care! I’d like to tell you a bit about us, how we came to start Clear Pathway Care and why we’re confident we’re going to shape the future of the care industry.
In 2016 I was approached by Jason, with an idea to create a new care company designed around modern care principals. They both love their roles in the care industry, despite the challenging nature of the work, because they were directly able improve the quality of life, for some of societies most vulnerable people.
There is evidence to suggest that people living in their own homes could deal with day to day challenges far better than people living in a residential care home. We believe that people are entitled to autonomy over their day-to-day lives and would be given the best changes of reintegration into society with under these circumstances.
Here at Clear Pathway Care we want to re-write the existing model to ensure all parties have a clearly defined pathway or progression. This is based on the transforming care agenda and we really believe people deserve to be a valued part of the community.
Our residents will be encouraged and supported to challenge themselves so that they can lead an unimpeded life. We have the view that everything is achievable and if the appropriate risk assessments have been completed, we will encourage our residents to pursue any activity they like, encouraging positive risk taking. We will also promote development and aim to reduce the level of funding necessary of each service user. We will also work with local employers to find our service users work, if possible, that would suit their individual needs.
While initial and on-going training will be provided for all our staff, they will also be encouraged to pursue additional training if any area of care particularly interests them, if a potential issue is identified and if they wish to be considered for a promotion within Clear Pathway Care. Rota’s will be organised to allow a thorough handover, between carers and additional time for training and development. We have invested in the best possible tools to facilitate staff being able to get on with providing care, while also being able to keep accurate record of activities and incidents. These include iCare, QCS quality compliance system and Rota Cloud. There will be various benefits to working for us such as childcare vouchers.
As we are working in partnership with Pivotal Housing Association, we will be able to move very quickly when obtaining new properties. This will often allow us to be able to offer local funding providers with the choice of several pre-selected homes so we can ensure the best possible environment is found for our service users. Our intention is also to set up individual centres of excellence, specialising in different requirements.
