Supported Living

Supporting living for adults with autism and learning disabilities

Our supported living service in Parkstone, Poole supports adults aged 18 and over, specialising in support for autistic people and people with learning disabilities, including individuals with complex support needs.

Supported living care that promotes choice, empowerment, and independence

At Clear Pathway Care, we believe supported living should be about much more than receiving care. It should enable people to have a home of their own, make choices about everyday life, build relationships, develop new skills and be part of their local community.

Our supported living service in Parkstone, Poole supports adults aged 18 and over, specialising in support for autistic people and people with learning disabilities, including individuals with complex support needs.

We start with the person, not the placement.

Before support begins, we take time to understand each person's needs, communication, routines, preferences, aspirations and the things that are important to them. Support is then built around the individual, helping them to live as independently as possible while receiving the right level of support to feel safe, secure and confident.

Support built around you

Everyone is different, so no two support packages should look exactly the same.

Depending on individual needs and goals, our team can provide support with:

  • Personal care and daily living
  • Developing independent living skills
  • Medication
  • Maintaining a tenancy and looking after a home
  • Shopping, cooking and household tasks
  • Accessing education, training or employment
  • Building and maintaining relationships
  • Hobbies, interests and social activities
  • Getting out and being part of the local community
  • Health and wellbeing
  • Communication and emotional support
  • Working towards personal goals and greater independence

Our focus is not simply on what someone needs help with today, but on what they would like to achieve in the future.

Understanding autism, learning disabilities and complex needs

Our experienced team specialises in supporting autistic adults and people with learning disabilities.

Where people require more individualised support, we focus on understanding the person, how they communicate and what may contribute to distress.

Our approach includes person-centred positive behaviour support, with an emphasis on developing communication, building practical coping strategies and supporting people to experience greater choice, confidence and quality of life.

Safeguarding, dignity and respect are at the heart of our service.

A Home, Not Just a Care Service

Having a home means having choice and control over everyday life.

We support people to develop and maintain their independence, make everyday decisions, enjoy their interests and routines, build relationships and become active members of their community.

Our role is to provide the support each person needs while recognising their individuality, rights, preferences and potential.

Working together

Good support comes from listening.

The person receiving support remains at the centre of decisions about their life and care. Where appropriate and in accordance with the person's wishes, we also work closely with families, advocates, commissioners, healthcare professionals, social workers and others involved in their support.

This collaborative approach helps us understand the whole person and provide consistent support as their needs, circumstances and ambitions develop.

Moving into supported living

We recognise that moving home or changing support arrangements can be a significant transition.

Before a placement begins, we work with the individual and those involved in their support to understand their needs and establish whether Clear Pathway Care can provide the right environment and support.

We want transitions to be carefully considered and centred around the individual, rather than expecting somebody to simply fit into an existing service.
